In Ohio, a Release of Lien form is used to release, cancel, or discharge a mechanics lien that has already been recorded. According to Ohio’s mechanics lien laws, a lien holder must release his lien within 30 days after the claim is paid. If they fail to file a lien release, they may be liable to the owner for any damages arising from the failure to release the lien.

• Claimant’s name & address
• Property owner name & address
• Property address and legal description (where the lien was filed)
• Date of original filing
• Recording/instrument number
