This is a Michigan Preliminary Notice for use on public works projects. This form can be filled out with a PDF editor or by hand after printing.
To preserve the right to file a claim against a payment bond on public construction projects in Michigan, anyone who didn’t contract directly with the prime contractor must send a Michigan Public Preliminary Notice to the general contractor. The notice should be served by certified mail no later than 30 days after the first date of furnishing labor and/or materials to the project.

Rules and regulations for sending a Michigan Preliminary Notice
Preliminary notice to the prime contractor required within 30 days of first furnishing labor and/or materials for those who did not contract with the prime contractor.
After completing the Michigan Preliminary Notice (Public Projects) Form, you must deliver it to the appropriate parties required by statute. Notices are typically served on the property owner and, for sub-tier parties, the general contractor. However, depending on the type of notice, it can be helpful to send notices to anyone else who is in charge of your payment, like a lender or surety company on the project.
