What does a(n) stand for on a sworn statement?

I'm filling out a sworn statement and it has a line after a(n) to fill in under contractor and I'm not sure what to put on that line

Sorry but I would have to see the form you are using to be sure. Generally I would opine that you either put the name of the general contractor OR the contractor you contracted with (if you are a sub-sub contractor or a supplier to a subcontractor) on that line.
