Hello! I am a subcontractor working under a contractor. I do not do construction work, but provide tree removal services. In order to get paid, I've been asked to provide a sworn statement (in MI) along with my other paperwork. I don't know how to fill it out? I had no subcontractors under me. Just my employees. Do I list them? They aren't under contract per se, so I don't know how to fill in the form.
Assuming you have paid your laborers fully, state NONE in the line where you are supposed to put UNPAID subcontractors and laborers. Have the statement notarized and submit it.
I concur with Jim and also mention that if you are asked to submit a Waiver of Lien before receiving money, make sure it is the form of "conditional waiver of lien" with your balance due inserted. Dont give a full unconditional waiver of lien unless money is in your hand!
