I am a contracting company in Florida who works in various states.
There are very few, if not any, state statutes that provide a lien release form. However, there is some general information that should always be included; such as the claimant's information, the property owner's information, the description of the property, and the lien filing information such as the date filed, the county/office it was filed in, and any recording number or book/page number. Additionally, the form will need to comply with the any county recorder requirements such as margin size, font size, etc.
You can download free templates for each state through our Construction Payment Learning Center Hub.
